If you're a new client, please complete the following forms and bring them to your first therapy session.

The Telemedicine Consent Form is in case you wish to have the option of a phone or skype session at any point during our work together.

If you have Medicare insurance, you must fill out the Medicare Non-participation Agreement and bring it to the first visit.
